With fall season in full swing, here are four important reminders to keep you on track:
1. Submit Player Registration Forms on eAYSO
As stated in National Policy Statement 3.7, effective Aug. 1, 2014, all AYSO Player Registration Forms must be submitted using eSignature in eAYSO. Regions are required to retain hard copies of any original hand signed Player Registration Forms for any player who isn’t registered using eSignature on eAYSO. Regions are required to keep these copies until the player reaches 24 years old to ensure compliance with laws regarding statute of limitations.
2. Forward all hand signed Player Registration Forms to the National Office
Please forward any original hand signed MY2015 Player Registration Forms, not eSigned in eAYSO, to the National Office. Please attach a control sheet found here and use a shipping method with a tracking number, such as USPS Certified Return Receipt, UPS or FedEx. Securely store and keep your other Player Registration Forms for now—we’re working on a process to collect those as well.
Regions are also required to send any original hand signed Volunteer Applications, not eSigned in eAYSO, to the Safe Haven Department. This signed copy is required to conduct a background check. Alternatively, a volunteer can log into eAYSO at any time and eSign the application to remove the red flag.
3. New eAYSO Feature
For Regions that prefer to print a copy of Player Registration Forms for their coaches to use for Emergency Treatment Authorization, eAYSO now offers the ability to print just the first page of the registration form. Parents should sign these single pages at an initial team meeting regardless of any other existing eSigned or hand signed form. The report is under Reports > Player and the report type is “Pre-Print Forms – Page One.” Every coach is required to have a signed copy of the Player Registration Form for every player at any AYSO activity. Please collect these forms from coaches and shred any duplicate copies at the end of the playing season.
4. Lost Player and Volunteer Registration Forms
We all know the importance of safeguarding player and volunteer registration forms. In the event that player or volunteer forms are lost or stolen, please notify Safe Haven immediately. We are required by law to notify the affected individuals of the data loss.
