New Requirement for Incident Reports
Turn in all Incident Reports to the National Office
AYSO Incident Reports are a fact sheet in the event something critical happens. Filling out an Incident Report immediately after an injury, property damage or any other serious incident will give you an opportunity to capture facts and contact information before witnesses leave the field.
Make sure all your coaches have several copies of the form at the beginning of the season, and know the correct procedure to fill out the reports and turn them in. Incident Reports are not only a best practice, but also required by AYSO to ensure proper follow up can made.
Remember, you must keep all incident reports on file and mail a copy to the National Office , ATTN: Risk Management, 19750 S. Vermont Ave., Suite 200, Torrance, CA 90502.
Incident Reports should be completed by any Region staff witnessing any serious incident involving, but not limited to:
- Threats of bodily harm
- Fighting whether or not a serious injury occurs
- Property damage
- Hospitalization of a participate
- Law enforcement summoned
- Service or notification of pending lawsuit
